Internship evaluation form: HEC sample criteria and how you are graded

An internship evaluation form is how your site supervisor tells your university how you actually performed. This page explains the HEC sample internship evaluation form, the 1 to 5 scale, how often it is filled, and how it feeds into your final internship grade.

Under HEC’s Undergraduate Education Policy, the internship is a degree requirement worth 3 credit hours, graded by a faculty member together with the field supervisor. That joint grading is why the evaluation form matters: it is the host’s half of your result.

Who fills which form

HEC’s internship guidelines describe three sample forms. Universities adapt them, so your department’s version may look different, but the roles stay the same.

Form Filled by How often (HEC sample) Purpose
Site supervisor evaluation Your supervisor at the company Every third week Rates your work and conduct
Student report form You (section A), faculty supervisor (section B) Every three weeks What you did, learned and struggled with
Activity log You, reviewed by supervisors Weekly, submitted every three weeks Tasks and hours, week by week
Final assessment Faculty supervisor At the end Combines all of the above into your grade

Some universities ask for more frequent evaluations. CUST asks for a site evaluation every two weeks, and UMT’s BS Education manual asks for a weekly one. IBA Karachi wants a sealed appraisal form within two weeks of finishing.

The HEC sample internship evaluation form

The HEC sample form for site supervisors records the supervisor’s name and designation, the host organisation, the intern’s name, the reporting period and the report number. It then asks the supervisor to rate the intern on nine points. In plain words:

# What the supervisor rates Scale
1 Punctuality: arrives on time 1 to 5
2 Respect for staff, policies and workplace norms 1 to 5
3 Understanding of the organisation’s work and willingness to learn it 1 to 5
4 Basic workplace skills 1 to 5
5 Professional conduct in all work situations 1 to 5
6 Initiative: looks for ways to contribute 1 to 5
7 Finishes tasks and reports back on time 1 to 5
8 Works well in a team 1 to 5
9 Reliability 1 to 5

The scale runs from 1 (does not meet expectations) through 3 (consistently meets them) to 5 (far above expectations). The form ends with the supervisor’s signature.

HEC’s guidelines also list the indicators the overall assessment should cover: work accomplishment, problem solving, organisational skills, communication, interpersonal skills and professional behaviour.

How your final internship grade is calculated

In the HEC guidelines, the faculty supervisor combines the parts with these weights:

Component Weight
Site supervisor evaluations 40%
Student reports 30%
Faculty supervisor evaluation 25%
Activity log completion 5%

The total is then turned into a performance level:

Score Level Meaning
80% to 100% Level 1 Outstanding, beyond workplace expectations
65% to 79% Level 2 High achievement
50% to 64% Level 3 Satisfactory
Below 50% Level 4 Not satisfactory

Two cautions. These guidelines were written for HEC’s 2020 policy, when the internship was non-credit; the current policy makes it 3 graded credit hours, and universities now set their own grading. Some, such as CUST, mark it pass or fail. So treat the weights as a common pattern, not your rule. Your department’s handbook is the final word. See HEC internship policy for the background.

The practical point holds everywhere: the site supervisor’s form is the largest single part, and the activity log is easy marks you should not lose.

Getting your evaluation form filled properly

  • Use your university’s form. Many universities, including CUST, IBA and NUML, use their own form. Get it from your internship office, not from a random template.
  • Hand it over in week one. Your supervisor should know from the start what they will be rating, and when.
  • Check the supervisor details. Name, designation and an official company email. IBA asks for official addresses rather than Gmail.
  • Do not fill it yourself. Universities can call or email the supervisor, and some ask for the form in a sealed envelope.
  • Keep your evidence in line. The ratings should match your log, your reports and your weekly reports. A glowing form beside an empty log looks wrong.
  • Keep a copy of every signed form you submit.

For employers: filling an internship evaluation form

If a student brings you a form, rate what you observed, not what would help them. Add one or two lines of comment with a concrete example: a task finished, a problem solved, a deadline missed. Sign it, add your designation and company stamp, and send it from your company email if the university asks for an electronic copy. Accrediting bodies such as NCEAC and PEC expect host reports to cover the training given, interest, work habits and punctuality, so comments on those are useful to the department.

How evaluations work at Ezitech

Ezitech is a PSEB-registered software house in Rawalpindi. Interns work on live client projects onsite or remotely, and a named mentor reviews their work every week. Those reviews and the intern’s daily check-ins with hours are recorded on our intern portal as they happen, not written up at the end.

Your university’s internship office can see this directly through our University Portal: tasks, check-ins with hours, evaluations and internship dates. If the department has its own form, the office uploads it there and we fill and sign it. The student does not have to chase anyone for a signature. Programme details and the fee are on the fee and refund policy page, and the tracks are on the internship programme page.

Questions

Who fills the internship evaluation form?

Your site supervisor at the company fills the main evaluation form. In the HEC model, you also complete a report form, your faculty supervisor scores it, and the faculty supervisor puts everything together for the final grade.

How often is the internship evaluation form filled?

In the HEC sample guidelines, the site supervisor evaluates every third week. Universities vary: CUST asks for a site evaluation every two weeks and UMT for BS Education asks for one every week.

What criteria are on the HEC internship evaluation form?

Nine points rated from 1 to 5: punctuality, respect for staff and policies, understanding of the organisation, basic workplace skills, professional conduct, initiative, finishing tasks on time, teamwork and reliability.

How is the final internship grade calculated?

In the HEC guidelines the weights are 40% site supervisor evaluations, 30% student reports, 25% faculty supervisor evaluation and 5% activity log. Your university may use its own scheme, including pass or fail.

Where can I get an internship evaluation form PDF?

From your own university internship office or department, because each university uses its own version. The HEC sample is in the annex of HEC internship guidelines and is a useful reference.

Can I fill my own internship evaluation form?

No. It must be filled and signed by your site supervisor. Universities can contact the supervisor or ask for a sealed form, and a self-filled form can get the internship rejected.
